Sir: We report the case of a patient who had both winter seasonal
affective disorder (SAD)1 and trichotillomania with winter
exacerbation, whom we treated with light therapy.

Case report: Ms. A, a 24-year-old woman, had a history of
trichotillomania since 10 years of age. From the onset of symptoms,
the trichotillomania persisted across the year, but improved
in the summer, when she needed no special head
covering, and worsened in winter, when she would habitually
develop a bald spot 5 cm in diameter.
