Sir: Patients affected by obsessive-compulsive disorder (OCD) often do not respond to specific pharmacologic treatments such as clomipramine and selective serotonin reuptake inhibitors (SSRIs) (approximately 40%-60% of cases). In up to 65% of this large subgroup of treatment-resistant patients, the addition of traditional neuroleptics appears to be useful, especially if comorbid chronic tics or schizotypal personality disorder is present. However, the long-term administration of traditional antipsychotics often leads to undesirable side effects such as tardive dyskinesia.
